I don't do the second application either. I used it again last night, but it didn't lather for me like you're able to get it to. We have extremely hard water though, so I'm wondering if that's why it won't lather for me no matter what I do. I'm just going to continue combing it through and letting it sit for 3-4 minutes, then rinsing and finishing off with an ACV rinse. I'm extremely happy with the results this way. So far I'm not breaking out as I'd feared I would, and my hair is much softer and healthier and the curls are laying much better, without causing my hair to lay big.
